Health professionals, human services personnel and schools can now download our referral form to efficiently coordinate outpatient counseling for those they serve. Our fax number is 715 – 504 – 8786.

However, adults and parents/guardians of minors are still welcome to contact us directly to learn about scheduling a session at New Insights Counseling. A referral from another provider is not necessary.
